The CW has released the synopsis for The Vampire Diaries: We All Go A Little Mad Sometimes (EP406), airing Thursday, November 15th, 8/7c. Highlight for spoilers:

ELENA’S NEW NIGHTMARE- Terrifying hallucinations leave Elena (Nina Dobrev) shaken and confused, causing her to make a dangerous mistake. After giving Stefan (Paul Wesley) some very disturbing news about Elena, Klaus (Joseph Morgan) takes matters into his own hands. Professor Shane (guest star David Alpay) reveals a story of ancient witchcraft to Damon (Ian Somerhalder) and Bonnie (Kat Graham). With help from Klaus and Stefan, Jeremy (Steven R. McQueen) begins a new chapter in his life. Finally, Elena makes a painful personal confession to Stefan, and Matt (Zach Roerig) gives Damon startling new information about Professor Shane.

The episode was written by Evan Bleiweiss and Julie Plec, directed by Wendy Stanzler.

     We think (and hope!;)) it is about Damon but this show always does what we do NOT expect that’s why I’m afraid it’s not about Damon;(

    I’ll just reiterate what I said in a previous post, even though S/E are my OTP, I’m okay if this is the episode where they break up. Even though I am going to be heartbroken for Stefan, it will move this triangle along which I feel is stagnant. Also, it will allow D/E to be explored (even though I can’t stand them as a couple, if they are actually going to happen it’s about time the writers start incorporating it more into the story), plus their fans deserve to have their couple for a while. Plus, I really do believe Elena will eventually realize how much she has taken Stefan for granted. I mean she has seen Damon with other women, yet Stefan has never shown any interest in another woman since the day he met Elena. When he finally does, I can’t wait to Elena’s reaction–even as a ripper Stefan didn’t care for other women so it’ll definitely add another dimension to the love triangle as a whole. Plus, if they break up this early in the season, it gives me hope that there is a good chance they will reconnect by the end of the season.

    Now my question is, is Elena’s painful confession about hurting someone else (like an innocent victim) or is it about Damon? And if it’s about Damon is she confessing feelings, or is she confessing something intimate she did with Damon?

    Yep they like to be tricky so better to be prepared for it to be something else just in case. She could simply confess to him that she fed on people at the party she went to with Damon and liked it. Given his desire for her to remain human, which she’s not, I doubt he’d handle that well.

    It is pretty vague. I’m wondering if the info Klaus gives to Stefan about Elena has something to do with her doppleganger status. We don’t really know much about that.

    Ancient witch craft discovery makes me think down the line little Miss Bennett finds out how to turn Elena back. Since Damon is in on it I get a concerned feeling that he’ll need to do something drastic like die for her to turn back since his blood turned her. I might be reaching but that thought has been nagging at me since they said Bonnie was going dark. I know he wouldn’t be permanently dead but still it would be sad.

    I wonder how Matt has dirt on the professor.

    OMG I think the confession is about Damon. I just hope she doesn’t cheat on Stefan! Why else would the confession be painful though? And I bet they break up in this episode. As much as I want my Delena together, I don’t think it’s right to cheat on Stefan (if she does). Like he deserves better than that! Everybody does. I’m sorry, but I’m not going to like this. It’s f–ked up if it’s true.

    The rest looks pretty good though. I cannot wait!! =)

    I agree she definitely shouldnt cheat on Stefan especially with Damon. That kind of ruins any DE relationship straight out of the gate. They should start on a fresh note…

    I dont think Elena’s reveal is about Damon, it might be something regarding her doppleganger blood… Or i like what someone else wrote about it being Bonnie turning her back to human with dark magic and Damon having to sacrifice himself. Now that would be new and interesting… (and of coarse Damon wouldnt really die) How cool would it be if he was turned human too!! :) secretly its what he wants most in the world…

    Does anyone else think Elena’s issues and hallucinations have anything to do with 2 doppelgangers (Katherine & Elena) living as vampires in the same timeline of present day?  Katherine being the oldest and strongest one of the two.  Maybe Elena’s hallucinations are things Katherine has already done.  Just a thought .. that is if the problem is not Tyler’s venom.

    i agree 100%.im damon fan and i want them together but not this way.but from what i read there will be steamy scene between damon and elena in ep 4 and caroline will see that which will change her relationship with elena.joshua butler who directed 
    ep 4 said ep4 will blow your minds(to delena fans).i dont like it it will ruin damon and elena and of course stefan always the good and the saint,damon a usual the bad the cheater.no no no

     I just wanted to thank you.  I am a Stelena fan and you are a Delena fan.  I wanted to say  that the comments you post are the most mature, respected and appreciated comments I have ever read.  You are the only Delena or Stelena that respects both actors and calls them on when they are doing something good or doing something bad or hurtful.  Love the way you step back and really look at the scene.   Look forward to more of you comments.

    I’m suspecting it has something to do with a werewolf bite, especially since “Klaus takes matters into his own hands.”  But there’s a really fantastic and well-written fanfic that deals with Elena having hallucinations of Katherine’s memories.  It’s called “Eyes of the Doppelganger,” and she’s got 17 or 18 chapters up so far.  You should check it out.

    Thanks Karen.  Obviously the episode tonight (Rager) appeared to be about the werewolf venom being put in the beer at Rebekah’s  party, but I’m not sure why Elena would still be hallucinating by the 6th episode considering Klaus healed her.  This is very strange.  Also, the Originals are not affected by a werewolf bite (according to Elijah) so I’m not sure what was going on with Rebkekah because she appeared to be having the same problem as Elena.

    Stefan was with Rebekah in the 20′s. So he has been with others during his ripper days. And I think, that despite Damon’s horndog ways, he was still rather chaste until he met Sage, who taught him to be another way while he waited for the women he loved (Katherine) and thought was locked in a tomb. Then not to mention the whole toying with his emotions that Katherine did to Damon. I don’t blame him for being a little wild with his “needs.” Guy had his heart ripped out. Not to mention, he falls for her doppleganger who turns out to be a better person than Katherine, but she chooses his brother (rightfully so, but still, ouch). The dude has had a rough life while Stefan spent most of his life blacked out on bloodlust. I think Damon should win. And I doubt Elana would cheat, it’s not in her, not even as a wild vampire. I think she’s going to confess her strong feelings for Damon. Stefan has already said “Its all for nothing if she’s in love with someone else.” So we’ll see what happens, but I’m all for D/E.

    I agree with your thoughts on the “triangle”, but I do have one quibble with this section: “I mean she has seen Damon with other women, yet Stefan has never shown any interest in another woman since the day he met Elena.” The fact that Damon has dated other women while loving Elena but never actually having been in a relationship with her (not to mention the fact that she was dating his brother at the time too), and Stefan not having feelings for other women while he was in a committed relationship (even when they were on the outs due to things beyond their control (i. e. Katherine/Klaus interference etc.)) with Elena are to entirely different things.  Correct me if I’m wrong, but what I take you to be saying is that as soon as Damon realized that he was in love with his brothers girlfriend, he should have not even tried to fight it by “dating” other women but kept himself single and even more miserable while he watched the woman he loved (whom to be perfectly honest never really started developing feelings more than friendship and maybe a little bit of attraction (but nothing serious) until the 3rd season) date/love his brother.  As for Stefan’s part, you seem to be saying it was a big thing that he DIDN’T get attracted to anyone else while he was dating the woman he loved who loved him back, even when they were not officially dating due to circumstances beyond their control back in season 2 they still both loved each other and neither was looking to date anyone else.  In season 3, again they still had a strong connection even though they had to be apart due to the circumstances; up until Stefan became so ashamed if his actions with Klaus and felt that she would never really accept him back and was so driven by revenge on Klaus that he tried to drive her away by being nasty to her (and everyone else too).  Those actions drove a wedge between Stefan and Elena and also served as a catalyst to bring Damon and Elena together as they bonded in trying to save Stefan first from Klaus and then from himself.  This wedge is further exasperated by Elena’s transition and need to feed on humans as seen so far in this season.  Up until this season there has been no real reason for Stefan to ever even think about getting over Elena and dating/loving another woman (well maybe in season 3 when he thought he’d lost her because of his ripper side/Klaus but we as an audience know better), and that actually still hasn’t come to pass yet as they are still very much a couple and in love.  Trying to compare one brother dating other women when he is single (if in love with someone else who has never openly admitted to feeling anything other than caring friendship to him) to another brother NOT dating anyone else while he was either dating or not dating the one he loves (who openly admits to loving him back and fought to get him back even when he doubted that she would) is a bit one-sided.  I think I’ll end this here as this post got exponentially longer than I intended and I’m not entirely sure I even got my point across in the rambling as it’s late and my brain is not fully functioning lol.
